Tearing of the meniscus is a very common injury for all ages, even people who are not active in sports or exercise. After the age of 25, most meniscus tears are degenerative in nature and is almost impossible to sew back together. Dr. Luks explains the significance of meniscus tear surgery and the impact that it might have on a patient's quality of life. Howard J. Luks, MD: One of the most common questions that I get in my office on a daily basis is "is surgery for my meniscus tear necessary?". A lot of that is based on your age and the type of tear that you have and most importantly the effect that the pain is causing, is having on your quality of life. Now age, if you are young, you typically sustain large tears and if you have a tear and you lose the meniscus then your risk of developing arthritis is very high. So, therefore sports medicine specialists are very aggressive in repairing or suturing and stitching back meniscus tears. That is typically possible and almost all tears and people under the age of 25. Over the age of 25, most tears are degenerative in nature where the tissue has simply worn out. Bladder Control Problems After Prostate Surgery

Bladder control or urinary incontinence after prostate surgery is very common. Fortunately after a matter of weeks, the majority of patients' issues resolve naturally. Dr. Jerry Blaivas, explains ways to deal with the issue during those weeks and what to do if the symptoms do not subside. Jerry Blaivas, MD: Urinary incontinence, which is not being able to control the urination, is very very common after prostate cancer surgery. Fortunately, in a vast majority of patients, over time that symptom gets better. So the treatment really depends on where the spectrum you fall. Immediately after the surgery, most people cannot control their urination and the best way to manage that for the short-term is to simply wear some kind of absorbent pads that you can get from any drug store. After a matter of weeks or months if it is not getting better, then we need to have a better understanding of what the causes are. Overwhelmingly, the most common cause of incontinence after prostate cancer surgery is what we call sphincteric incontinence. Sphincteric incontinence means that the muscles that were in the wall of the prostate that ordinarily kept the urethra closed, have either been completely or partially removed or damaged. In this video, Dr. Blaivas explains diagnosis, treatment, and other ways to deal with the common problem of urinary incontinence in prostate cancer patients. Urinary incontinence after prostate cancer is usually based on the sphincter muscle not working properly. In about 5% of cases, the patient's bladder contracts involuntarily. If the condition does not go away over time, surgery to correct the incontinence may be required. Surgical options include a sphincter prosthesis, which replaces the sphincter itself with an implanted device, and a sling procedure, which is mesh placed under the urethra so that it is compressed during activity. So the best way to communicate what your symptoms is to your doctor or to start with, by paying attention. So pay attention to your own symptoms and try to anticipate what kinds of questions the doctor is going to answer. The best way to pay attention to your own bladder symptoms is to keep a bladder diary and you can do that without having seen a doctor. There is places you can find diaries online or you could just make up your own and the most important thing in this diary is just for one 24-hour period is to record the time and the amount of each urination and any symptoms that happen either during the urination like pain or before the urination like difficulty starting or incontinence or between urinations such as incontinence when you cough or sneeze so, and whenever your symptom is, pay attention to when it happens. Urination In The Middle of the Night Explained by Expert Urologist

Dr. Blaivas explains that getting up in the middle of the night to urinate is called nocturia. Reasons for nocturia include the bladder not holding enough to allow the patient to sleep through the night, and the patient's kidney potentially making more urine than the bladder can hold. Nocturnal polyuria is when too much urine is produced overnight, therefore causing the patient to get up in the middle of the night to urinate. Jerry Blaivas, MD: Getting up in the middle of the night to urinate is called nocturia. Nocturia is very very common, in fact most people get up once or twice at night to urinate starting in the teenage years even. If you think about it, there is only two possible reasons why someone would get up in the middle of the night to urinate. One is that their bladder does not comfortably hold enough to allow them to sleep through the night, and, two, you make more urine, that it is to say your kidney is making more urine then your bladder cannot comfortably hold overnight. The second condition, we called nocturnal polyuria. The nocturnal means nighttime and poly means too much and uria means urine. So nocturnal polyuria means that you make too much urine overnight and that is probably the commonest cause of nocturia. The most common cause of that even is just drinking too much before you go to sleep.
